What Is It?

The Herbst-style sleep appliance adapts the orthodontic Herbst telescopic mechanism for OSA treatment. Bilateral telescopic arms provide rigid, continuous mandibular advancement — ideal for moderate to severe OSA requiring 6-10mm of advancement.

Components

• Upper Tray — Acrylic baseplate with Adams clasps on molars • Lower Tray — Acrylic baseplate with Adams clasps on premolars • Bilateral Telescopic Arms — Stainless steel tubes and plunger rods • Elastics (optional) — To prevent mouth opening during sleep

How It Works

Bilateral telescopic arms lock the mandible in a forward position. Unlike elastic-strap devices, the Herbst mechanism provides rigid, continuous advancement — beneficial for severe OSA cases. The telescoping action allows mouth opening while maintaining protrusion. Optional vertical elastics prevent mouth opening.

1. Receive intraoral scans and protrusive bite (6-10mm) 2. 3D-print working models 3. Fabricate upper and lower acrylic trays with Adams clasps 4. Install telescopic arms — tube on upper, plunger on lower 5. Verify arm length and pivot alignment 6. QC: Arm function, clasp retention, acrylic finish
