What Is It?

The Herbst Appliance, originally developed by Emil Herbst in 1909 and repopularized by Pancherz in 1979, is a fixed functional appliance using bilateral telescopic tube-and-rod mechanisms to continuously hold the mandible forward. Because it is cemented, it eliminates patient compliance issues entirely.

Components

• Bilateral Telescopic Arms — SS tube (upper) and plunger rod (lower) on each side • Upper Crowns/Bands — SS crowns on maxillary first molars • Lower Crowns/Bands — SS crowns on mandibular first premolars or canines • Lower Lingual Arch — 1.2mm SS wire connecting left and right lower anchorage • Upper Transpalatal Arch (optional) — 1.2mm SS wire • Pivot Screws — Secure telescopic arm connections

How It Works

The telescopic arms create a rigid connection between maxilla and mandible. When the patient closes, the mandible is forced forward. The arms telescope during opening/closing, allowing near-normal function. Provides 24-hour mandibular protrusion — treatment duration 6-9 months.

Digital Workflow

1. Receive intraoral scans + construction bite (7-8mm advancement) 2. 3D-print working models 3. Fit preformed SS crowns/bands on printed model 4. Solder telescopic arms to crowns — verify arm length and pivot alignment 5. Solder lingual arch and TPA wires 6. Polish all solder joints and crown margins 7. QC: Arm telescoping action, crown fit, weld integrity

• Crowns: Preformed SS (0.15mm), ISO 22743 • Arms: Medical-grade SS telescopic tubes, 18/20/22mm lengths • Lingual Arch: 1.2mm ASTM F899 SS, 1mm tissue clearance • Solder: Silver-based dental solder, 700-800°C • QC: Arm function test, crown marginal fit, solder inspection (10×)
