The Twin Block remains the workhorse of functional appliance therapy. Bite registration is critical: advance the mandible 5-7mm with 3-5mm vertical opening. The 70-degree incline angle is non-negotiable: angles below 45 degrees lose mechanical efficiency; above 80 degrees cause excessive vertical opening. Use heat-cured acrylic for occlusal blocks. Adams clasps on first molars (0.8mm wire) with ball clasps between premolars provide the most reliable retention. For midline fractures, reinforce the screw area to minimum 3mm thickness. Lower incisor proclination? Ensure lower acrylic does not contact lingual incisor surfaces.
