Quad Helix and TPA: Versatile Fixed Appliances for Transverse Control

The Quad Helix uses 0.038-inch stainless steel wire with four helical loops to deliver slow, continuous expansion at approximately 2mm per month. The helices act as springs, storing energy upon activation and releasing it gradually with no patient compliance needed. The TPA (Transpalatal Arch) is a 1.0-1.2mm wire crossing the palate, soldered to molar bands. It serves multiple functions: anchorage reinforcement (prevents mesial molar drift), molar rotation (via omega loop activation), mild expansion (1-2mm), and space maintenance. Both appliances are cemented, eliminating the compliance variable. Key design tip: Quad Helix requires 4-6mm of activation beyond passive width; TPA should maintain 1-2mm clearance from palatal tissue.
